The files on this web site are provided to assist you in establishing a sound administrative foundation for your ministry.  Various Calvary Chapels and Church Administrative Services provide this information for the use of all Calvary Chapels.  The intention is that you copy these files and change them to make them applicable for your church.

All files are in either Microsoft Word or Excel format.  The files are organized by categories such as Ministry, Office, Tax Issues, etc.

There are three forms in the Personnel Folder (Performance Appraisal, Request for Status Change, and the Personnel Memo) that say that they are “Right Protected” when you open them.  There is another in the Ministry Folder (Counseling Forms).  Open them as “Right Protected” make any necessary changes and save them to another file name.  This will retain the changes.  Each time that you open them you will be required to click ”Read Only”.  It is a little cumbersome, but until I get an unrestricted version that is the best that I can do.

The files containing Articles of Incorporation and Bylaws are samples taken from the Articles of Incorporation and Bylaws of a Florida Calvary Chapel.  The areas that are Florida specific or require change to make them your own are noted in parenthesis.  Once you complete you Articles of Incorporation and Bylaws they should be reviewed by an attorney in the state that you are operating to insure that they conform to the laws of that state.

The Children’s Ministry Parents Guide is formatted so that if printed in landscape and folded in half it will create a booklet.

The Key Request and Request for Time Off Forms are printed two to a page.
